Filling these forms prior to your appointment can save you time in the waiting room and may expedite your appointment. The completed forms can be filled out online and submitted, or filled out and faxed to 985-625-2206, or dropped off at our Thibodaux, Franklin, or Laplace clinis prior to your appointment. Please avoid mailing paperwork, as it may not be received in our office prior to your scheduled appointment.
